The Cardiac Rehabilitation Service provides a City-wide, evidenced based, individualised approach to support patients and their families/carers who have had a cardiac event in the previous 12 months.
Cardiac Rehabilitation supports people and their families with heart disease to return to a full and active lifestyle, by helping them to understand and self manage their condition, lifestyle and medications and support their social, physical and mental well being.
In hospital visit to patients following a heart attack and/or angioplasty/ stent at Salford Royal.
After discharge a choice of Telephone call and/or home visit, available to all patients referred to the service.

6-week programme of exercise, health education and relaxation in the hospital, community or at home

6-week stress management programme helping to understand stress and develop coping strategies

8-week programme of healthy eating advice, physical activity and support to motivate change for weight management.

Psychological, healthy eating and other lifestyle advice on a one to one basis

Ongoing telephone and/or home visit support
Referrals are made following admission to hospital, or a cardiac event within the last 12 months.
