The Community Heart failure Service provides high quality localised care for patients diagnosed with left ventricular systolic dysfunction (LVSD) or severe inoperable (mitral or aortic) heart valve disease.
The service is delivered in community based clinics or through home visits for patients with advanced heart failure symptoms and inoperable heart valve disease.
