The Discharge Assessment Service supports the safe discharge of patients who require community care; this can be direct to the patients own home with District Nursing or carers support, twenty-four hour care, or to a residential/nursing home for assessment and ongoing rehabilitation.
The service also provides

support to people within their own homes following a period of time in hospital.

assessment and rehabilitation to aid people to recover previous functional ability following surgery, illness or injury.

promoting health, well-being and independence supporting them to remain in their own home.
