This Intermediate Care Bedded Units provide in-patient nursing, rehabilitation and assessment in order to prevent an unnecessary hospital admission or promote early discharge from hospital, and prevent premature admission to long term care.
There are three Intermediate Care Units:

Heartly Green and

The Limes
Each provides rehabilitation and facilitation of early discharge in a residential environment. Swinton Hall and Heartly Green can also provide for additional nursing need to prevent unnecessary hospital admission.
Swinton Hall
Swinton Hall Intermediate Care unit is an 18 bedded in patient facility integrated within a nursing home. The beds are made up of rehabilitation beds, nursing/crisis intervention and assessment beds.
Heartly Green
Heartly Green Intermediate Care unit is a 29 bedded in patient facility integrated within a residential home. The beds are made up of rehabilitation beds, nursing/crisis intervention and assessment beds.
The facility is split into two units: 14 bedded unit and 15-bedded unit.
The Limes
The Limes Intermediate Care unit is a bedded in patient facility integrated within a residential home. The beds are made up of rehabilitation beds, crisis intervention and assessment beds.
Referrals
Referrals are received from a wide variety of sources including GP’s, Hospitals, Social Workers, Community Matrons.
