Cancel or request to rebook your outpatient appointment

Please use the form below to cancel or rebook your appointment in Outpatients.

 

Please note: 

If you have a new patient appointment booked through the electronic booking system called NHS e-referral and need to cancel this or rebook the appointment, you need to contact the national telephone appointment line on 034560888888 between 8am to 8pm weekdays, and 8am to 4pm weekends and bank holidays. The appointment line is closed on Christmas Day.

 

If you are cancelling your appointment and have not indicated that you need a further appointment, we will discharge you back to your GP unless contact is made with the hospital within two weeks of your notice of cancellation.

 

If you request to rebook an appointment, we will generally be in touch in writing with a new date and time, so please specify in the comments section below any preferences you have for your new appointment, and we will do our best to accommodate them when rebooking.
