Salford Together

Integrated care is all about providing better care and support to our patients, users and the people of Salford by co-ordinating our services more effectively, with the focus on improving the health and wellbeing of people across the city.

 

Salford Together is a partnership between NHS Salford Clinical Commissioning Group, Salford City Council, Salford Royal and Greater Manchester West Mental Health NHS Foundation Trust.

 

Together, this partnership is working to integrate health and social care for older people in Salford, bringing the contributions of GPs, District Nurses, social workers, mental health professionals, care homes, voluntary organisations and local hospitals into a more aligned system and provide older people with the support they need to manage their own care.

 

There are proposals to create a new Integrated Care Organisation (ICO) to bring together adult social care and health services by:

 

  • Transferring the provision of many Salford adult social care services to Salford Royal

  • Transferring the contracting for Salford adult and older adult mental health services to Salford Royal to contract directly with Greater Manchester West Mental Health NHS Foundation Trust
